20 Professional Practice Guide Absolute contraindications:⁴ • Recent changes on resting ECG • Unstable angina • Uncontrolled cardiac arrhythmias that are symptomatic or cause hemodynamic compromise • Symptoms of uncontrolled heart failure • Symptomatic or severe valvular disease • Acute pulmonary embolism • Acute myocarditis or pericarditis • Suspected or known dissecting aneurysm • Acute systemic infection PAD-specific absolute contraindications: • Rest pain in the affected limb • Critical limb ischemia or indication for surgery • Skin lesions of the lower limbs Relative contraindications:⁴ • Left main coronary artery stenosis • Moderate valvular stenosis • Electrolyte abnormalities • Severe resting hypertension (> 200/110 mmHg) • Tachyarrhythmias or bradyarrhythmias • Hypertrophic cardiomyopathy (or other forms of outflow tract obstruction) • Neuromuscular, musculoskeletal, or rheumatologic disorders exacerbated by exercise • High-grade atrioventricular block • Poorly controlled metabolic disease • Chronic infectious disease • Physical or mental conditions resulting in an inability to exercise safely 4. Lifestyle and Physical Activity Management Special considerations for patients with diabetes and/or peripheral neuropathy: • Wear properly fitted footwear to prevent skin irritation or the development of wounds. • Perform daily self-examination of the feet. • Patients who are amputees or who require walking assistance should be referred to an appropriate specialist. People living with PAD are at a high risk of cardiovascular disease.
